Our doctors are in-network with several insurance panels. When making your appointment, we will do our best to ensure that you are matched with the best provider based on your needs, as well as your insurance plan. For insurance companies not listed, we may be able to provide services at the out-of-network rate.

Please note: your mental health benefits may be administered by a company other than your major medical insurance carrier. For all cases, please contact your insurance company to confirm coverage and network status.  Our insurance verification staff will also be contacting your insurance company to obtain your benefit information; however, it is still recommended that you also contact your carrier to review your coverage.

HMO Plans

HMO groups typically require a referral for visits to a specialist. Please note that the patient is responsible for obtaining this referral.  Please contact your PCP or your HMO plan administration to obtain the appropriate referral prior to your initial appointment and ask it to be faxed to 630-424-9017. If no referral is on file, the patient will be responsible for the full balance of the initial appointment.

Pre-Certifications and Referrals

A majority of insurance companies will require a referral or pre-certification for the psychological/neuropsychological testing itself. Our office will coordinate with your insurance company to obtain the appropriate authorization for the testing after the initial appointment has been completed. 

Billing and Payment

You are welcome to meet with our practice manager or finance administration staff to review your coverage to assist in determining your out-of-pocket costs or to discuss payment plan options.

As a convenience to our patients, we are happy to bill your insurance directly on your behalf.

All co-pays, as dictated by your plan, are required to be collected at the time of service.
